select the exam you are requesting and fill out the form.
Our Patient Services Coordinator will contact you by phone with a day and time for your appointment.
To ensure accurate scheduling and excellent patient care, we require a copy of your order form you received from your doctor. You may take a picture via your smartphone and email it to firstname.lastname@example.org

To expedite your check-in process at the time of your appointment, please bring the following:
Drivers License/Identification Card and Insurance Card.
Optionally, you can complete the following forms and print them to bring with you to your appointment:
Patient Registration EHR, Medical Records Release, Policy Disclosure: Pre-Registration Forms
Patient Portal for appointment information and your health records.
If you are trying to obtain information about a recent visit, please note that it may take one or two business days for your records to appear in your account.**You will need to enter your PIN if you have not already established a link between your Health Companion account and Radiology Associates
